Employee's neck burned when steam explodes from laminator

At approximately 11:00 a.m. on April 3, 1990, Employee #1 was standing about 6 ft from the front southeast corner of a newly-installed laminator machine, looking at the new equipment. The laminator was not operating at the time, except that the steam flow into and out of the machine was being tested. Steam was fed into the machine through pipes from a boiler at a maximum of 100 psi. The steam line piping and associated control valves were apparently complete and functional. A steam-filled front roller of the machine exploded when the inner shell of 5/8 in. steel ruptured from the seams, spewing a large volume of steam to the sides. Employee #1 suffered first- and second-degree burns on his head. Employee #1 was a foreman in the production area, but had participated in installation of the laminator.
