Employee's finger amputated in belt and pulley drive

Employee #1, a maintenance mechanic, was checking the wear on the belt and pulley drive of a pump that drove a blower in a large flour mill. The power to the pump was shut off, but not locked out. When the power was turned back on from another location, Employee #1's right thumb was pulled through the belt and pulley. It was amputated above the first joint. Upon investigation, it was determined that Employee #1 had not been present when coworkers were trained in lockout/blockout procedures.
