Employee injures back when caught in decaser

At 5:45 a.m. on November 15, 1991, Employee #1, a 48-year-old employee of Owens Brockway, was working near a Unimatic uncaser. He encountered a problem in the conveyor and climbed the decaser to clear a few bottles. The decaser was activated and Employee #1 got caught under its suction cups. He sustained a strain and contusions on his back and neck and was taken to Los Angeles County Hospital, but was not admitted. A violation was noted and the employer promptly abated the violation. The employer was advised to review the equipment maintenance procedures and to oversee the employees' performance on the job to ensure their safety and health.
