Investigation Summary
At approximately 12:30 a.m. on April 23, 1996, Employee #1, of the Connolly-Pacific Company, was cleaning up his work area at the end of his shift. Earlier he had been welding a metal frame on the bed of CAT truck #773B. A coworker found Employee #1 lying on the concrete floor behind the CAT. First aid and CPR were administered, and 911 was called, but Employee #1 died. An autopsy found the cause of death to be a heart attack.
