Employee breaks back in fall from roof of tank

At about 8:30 a.m. on April 16, 1992, Employee #1 was operating a portable grinder on the roof of a 48 foot tank under construction. Employee #1 then fell 48 feet from the edge of the tank and landed on his back on the ground. He was wearing a safety harness and lanyard, which he failed to connect to a substantial member or a securely rigged line. Employee #1 sustained a broken back, for which he was hospitalized.
