Employee's fingers cut when caught in v-belt and pulley

At about 10:15 a.m. on February 26, 1993, Employee #1 was changing the belts on a coke handling machine. The machine was energized and Employee #1's left hand was caught in the v-belt and pulley. He suffered cuts to his fingers, for which he was hospitalized.
