Employees fall through unsecured floor opening

At approximately 8:15 a.m. on September 16, 1992, Employee #1, of C.W. Driver Contractors, was conducting a clean-up operation on the second floor of a patio area at a new science building. The employee picked up a 4 by 8 section of plywood that was not labeled "Do not remove--floor opening," and that was not secured in place by nails to prevent it from being accidentally displaced or removed. Employee #1 accidentally stepped through the open floor hole and fell 14 ft to the floor below. He was taken by paramedics to St. Mary's Hospital in Long Beach, CA, for treatment and observation.
