Investigation Summary
Employee #1 and two coworkers were moving into storage a 300 lb spider tool, which is a material handling device used to lift and support the fuselage of an aircraft. The tool was designed by the employer. Employee #1 was pulling the tool while his coworkers were pushing it. In the process, a wheel rolled over the side of his left foot, fracturing it. Causal factors in this accident were that Employee #1 should have been pushing rather than pulling, and the spider tool was being moved at an unsafe speed.
