Investigation Summary
At approximately 3:55 p.m. on April 12, 1995, Employee #1 was helping a forklift truck operator unload a stack of wooden poles into the company's open yard. When the lift operator drove the truck forward a few feet to pick up a pole in a front end loader, Patrick model #ARR10, serial #05932060, he saw Employee #1 about 60 ft away on his right hand side. After the lift operator picked up the pole, he backed up the truck and saw Employee #1 on his right side, close to the truck. The truck hit a pothole, causing the end of the pole to swing and hit Employee #1 on the head and shoulder. Employee #1, whose head was protected by a hard hat, suffered a fractured shoulder. He was taken to Long Beach Medical Clinic. The accident's causal factors include the operator's failure to make sure that the surroundings were clear before backing up, and the potholes.
