Employee's fingertip amputated by falling machine part

Employee #1, who works for the manufacturing division of a multinational printing house, was adjusting a 20 lb print head scanner on bindery line 130. The scanner is suspended from a bracket by three machine bolts or cap screws, and reads labels on a magazine conveyor fitted with metal lugs that protrude upward. When the mounting bolts were loosened to permit adjustment, the scanner came loose and fell downward, pinching off Employee #1's fingertip, which was near the conveyor. The bracket design had changed three times and no new job safety analysis or procedures were in writing. Employee #1 had verbally reviewed the bracket functions. The supervisor was not in the area. No restraining devices on the print head conveyor line prevented the print head from falling. The chain of command includes the main bindery 130 operator, manufacturing supervisor, and plant manager. Production is deadline driven.
