Investigation Summary

Investigation Nr: 170716617
Event: 01/30/1992
Employee killed in fall from oil well servicing rig

At about 10:24 a.m. on January 30, 1992, Employee #1 was a member of a crew servicing oil well #87Z at Chevron Cymric 8Z lease. Employee #1 had climbed to the rod transfer stand (pickle board), which was about 64 ft above the ground. The head well puller and another well puller were on the ground at the well head attempting to stab a rod socket into the production "T" (pipe fitting). The well puller heard a metal hard hat hitting the mast as it fell. The well puller looked up and saw Employee #1 falling about 20 ft above. The well puller yelled a warning, and both he and the head well puller ran. The head well puller ran about 5 ft to the east when he was struck by the falling Employee #1, and knocked to the ground. Employee #1 suffered extensive deceleration injuries to his vital organs. He was pronounced dead at the Westside District Hospital, in Taft, CA.
