Employee injured in fall when ladder slid

On November 13, 1993, Employee #1 had climbed a 13 ft Keller extension ladder, model #2166, to a 9 ft storage area to retrieve a box. The ladder slid from underneath him and he fell. Employee #1 sustained six fractured ribs and multiple facial fractures.
