Investigation Summary
Employee #1, age 20, was part of a framing crew that was working on the fourth floor roof. Three runs of 4 ft wide plywood had been nailed in place, one along the south perimeter, and two along the north perimeter. The second run hung approximately 12 to 14 in. over the east roof edge, and the third run hung approximately 6 in. over the same edge. Employee #1 was standing on the second run, reaching out to place a small (48 in. by 55 in.) piece of plywood that had been left out of the first run. A coworker said that he saw Employee #1 lose his balance and ran to help him, but that he was unable to reach Employee #1 before he fell 55 to 60 ft to the ground. Apparently, Employee #1 briefly clung onto the second run of plywood before losing his grip. Paramedics from the Los Angeles City Fire Department responded and transported Employee #1 to the Queen of the Angels Hollywood Presbyterian Medical Center, where he died.
