Investigation Summary

Investigation Nr: 170153613
Event: 01/14/1991
Employee injured in 22 ft fall from scaffold ladder

At approximately 10:41 a.m. on January 14, 1991, Employee #1, a carpenter for Brand Scaffold Services, Compton, CA, was climbing a scaffold ladder that was attached to the north leg of a steel tubular scaffold at the Chevron refinery in El Segundo. He was at a height of approximately 22 ft when he lost his hard hat and tried to grab it as it dropped. He apparently lost his hold and fell, because he was next seen lying on the ground to the west of the ladder. Paramedics from the El Segundo City Fire Department responded and transported Employee #1 to the Harbor UCLA Medical Center in Torrance. He was treated for a fractured right leg, broken ribs, a collapsed lung, and a severe concussion.
