Investigation Summary
Employee #1 was working on a D-5 Flexseal Core, with engine lathe #27. He placed his right index finger inside the core to feel the surface, which was thought not to be cutting cleanly. He accidentally put it on the grinding letter wheel, severing his finger just past the first joint.
