Employee's hand struck by moving part of equipment

On May 13, 1992, Employee #1, of General Foods, was repairing equipment when his hand struck a moving part and he sustained a cut or laceration to his fingers. The employee maintains that he locked out the machine. During the investigation, the machine was tested with the appropriate controls off and the jaws would not move. It was concluded that Employee #1 did not isolate the part of the equipment that struck him. The employer has a lockout procedure and the employee said that he was familiar with it.
