Investigation Summary
At approximately 9:16 p.m. on August 17, 1992, Employee #1, of Industrial Wire Products, was inspecting the wire quality on wire draw machine #D-9 while it was running. He reached the in-running wire through an opening by the machine frame and his right fingers got caught in the nip point. His right middle finger was amputated at the first digit and the index and ring fingers suffered deep lacerations. Employee #1 was hospitalized at San Antonio Community Hospital, Upland, CA. The machine was not adequately guarded and the employer did not implement the compliance and communication program in substantial compliance. Employee #1 had worked as a machine operator for the employer for 2 years and 8 months.
