Investigation Summary

Investigation Nr: 170152771
Event: 02/21/1992
Employee injured while securing load to flatbed truck

At approximately 11:15 p.m. on February 21, 1992, Employee #1, a truck driver for Higashi Farms, Inc., was securing his load to his flatbed truck. Although the ground was dry and in good condition, Employee #1 slipped while pulling down the load line with his feet and hands. The line slipped from his hands and forced him to the ground. Suffering a minor head injury, Employee #1 was taken to the St. John's Hospital emergency room, where he was treated and released. The employer admitted to not periodically inspecting the driver's workplace to determine unsafe work practices, but has started as of February 21, 1992.
