Investigation Summary
At approximately 10:00 a.m. on September 14, 1992, Employee #1 had just come to work when she was assigned by her supervisor to help fix a problem with the in line vacuum former. This machine makes plastic seedling trays for vegetables and was created by the owner about 10 years ago. A roll of plastic sheeting about 2 ft wide by 1/8 in. thick is fed into the machine, which heats it up. It then enters a press to form a tray with about 1 in. square by about 1 1/2 in. deep areas throughout. It is then transferred to the next press, where small holes are punched in each of the plant areas. At this point the tray sometimes sticks to the press. Even when there is no problem, the operator is required to place her hand in at this point to remove the tray. There was no guard or other device to prevent the operator's hand from entering the point of operation. Neither press on this machine is guarded. The first press is hot and the operators have received many burns on their arms. While Employee #1 was trying to remove the tray on the second press, the press came together (the upper descends and the lower rises in about 2 seconds) and caught her three left fingers and amputated them. A yellow tag was hung on the machine and help was given on various ways to guard the machine. Employee #1 has operated this machine for about 5 years.
