Employee injured when head struck by flying metal disc

Employee #1 and coworkers, of Anderson Pump Company, were capping a water well following the removal of the pump system and downwell shafting. The lead person stuck the head of an oxyacetylene torch into the well head to check for flammable vapors and gases, with negative results. An 18 in. diameter disc plate was being trimmed and tack-welded to the well casing when the torch flame or some hot slag ignited a build-up of flammable vapors or gases. There followed an explosion that sent the partially secured cupping plate flying into the side of Employee #1's head. He suffered severe head trauma and was hospitalized.
