Employee catches index finger in grinding machine

At about 7:50 a.m. on September 24, 1993, Employee #1 installed a 16 inch diameter feed wheel in Cincinnati Grinding Machine #3-1, set it in motion, and dressed it to make the circumferential surface parallel to the wheel spindle. The wheel had a notch or slot at a point on its circumference to release ground parts. Apparently Employee #1 pressed the circumferential surface with his left index finger, with his finger held parallel to the notch axis. His finger fell into the notch, turned with the wheel, and was nearly amputated. Employee #1 was taken to the Downey Community Hospital and to the Western Hand Center. The finger could not be saved and was amputated. A serious violation of 8CCR3314(a) was observed and cited.
