Employee's ankle fractured by pallet jack

According to the employer's report, on November 2, 1991, Employee #1 was hit by a battery-operated pallet jack after he stepped off it while it was moving. He sustained three fractures to his left ankle and was hospitalized. The employee's residence was visited on November 25, 1991, and a business card was left. He did not call and there was no answer when calls were placed to the employee's house. No defects were found when the pallet jack was checked soon after the accident. No violation related to the accident was found.
