Employee killed in fall from ladder

At approximately 9:30 a.m. on February 1, 1991, Employee #1 was climbing a 10 ft ladder. He was holding onto the rungs with one hand and carrying paint in the other hand. He was at a height of approximately 6 ft when he slipped, lost his balance, and fell, striking his head on the ground. He got up, but was told to sit down and await the arrival of paramedics. During the 8 to 10 minute wait, Employee #1 passed in and out of consciousness. He was transported to the hospital, where he died three days later. The ladder was secured; Employee #1 should not have attempted to climb it with one hand.
