Employee's hand crushed in aluminum tube reducer

Employee #1 was operating a #350 aluminum tube reducer when he saw a piece of scrap metal wedged in the grips. He reached into the 6 in. collar opening, where the reduced aluminum tubing comes out, to retrieve the scrap before the next aluminum tube started moving out. As he pulled out the scrap, his elbow momentarily bumped the inside edge of the collar. He straightened his right arm and extended his hand back into the grip area just as the next aluminum tube was coming out of the reducing area. His right hand became wedged between the outgoing tube and the inside of the collar housing. It was crushed and lacerated, requiring reconstructive surgery. Employee #1 failed to shut down the machine before attempting to remove the scrap metal.
