Investigation Summary
At approximately 2:00 p.m. on April 24, 1992, Employee #1 was operating a triple roll pin cutting machine that was cutting a set of small pins. The employee suddenly reached into the unguarded side of the cutter to clean some parts when it cut her little fingertip, amputating it. She was hospitalized. No defects were found with the new machine or with the procedures and safety precautions. The employee was properly trained with work experience. The employer was found in violation of T8CCR 4002(a), and a serious citation was issued with penalty.
