Investigation Nr: 170717722
Event: 04/14/1992
Employee injured when struck by forklift

Employee #1 was walking from his work station to the washroom and had to cross a main aisleway. He said he looked for traffic but there was sunlight coming through the skylights which might have hindered his vision. As he crossed the aisle, Employee #1 was hit by a forklift coming from his right. The truck was running in reverse and was carrying a 5 ft by 5 ft bin. The forklift knocked Employee #1 down, breaking his right ankle and four left toes. The forklift was operating in a safe manner and was not speeding. Employee #1 did not see the oncoming vehicle and walked directly into it.
