EMPLOYEE STRUCK BY FORKLIFT

AT ABOUT 7:00 PM ON APRIL 6, 1992, AN INJURY OCCURRED INSIDE THE BOTTLING DEPARTMENT OF A BREWERY IN VAN NUYS, CA. WHILE WALKING TO A LUNCH ROOM, EMPLOYEE #1, A BOTTLER, WAS STRUCK FROM BEHIND BY THE BLADE OF A FORKLIFT TRUCK. THE FORKLIFT TRUCK WAS EMPTY AT THE TIME AND THE BLADES WERE IN THE LOW POSITION. PARAMEDICS TRANSPORTED HIM TO NORTHRIDGE HOSPITAL, WHERE HE WAS HOSPITALIZED WITH FOOT AND ANKLE INJURIES.
