Employee's fingers amputated in sprocket drive

Employee #1, of C and M Fine Pack, Inc., was operating a lift elevator used to raise scrap plastic to the feed opening of a regrinding machine. The elevator was driven by a chain and sprocket. During the previous shift, the sprocket had worked loose from the shaft and the operator had removed the guard to reinstall the sprocket. After the shift change, Employee #1 continued to operate the machine without replacing the guard, periodically checking that the sprocket had remained in position. He was pushing the sprocket back onto the shaft without stopping the machine when his right hand became caught. His fingers were amputated.
