Employee injured when arm caught in rollers of glue machine

At approximately 10:20 a.m. on April 2, 1991, Employee #1, a maintenance mechanic, was assigned to clean the rollers of a machine that applied glue to both sides of an insulating product prior to attaching metal sheets. Employee #1 had the machine running so he could apply acetone solvent to the rollers while they were turning. As he was applying the solvent, his right hand became caught in the rollers and his arm was pulled in up to his elbow. Employee #1 failed to use the roller adjusting part of the machine to open the rolls to their 9 in. wide, full open position.
