Employee injured by flying hose connector

At approximately 11:30 p.m. on March 29, 1991, Employee #1 and a coworker were unloading liquid carbon dioxide from a tanker truck into a bulk supply tank at the North America Chemical Company. The coworker connected the hose to the tank trailer. When the trailer valve was opened to begin transfer, the female coupling was violently ejected from the male connection on the truck and struck Employee #1 on the leg. He sustained injuries that required hospitalization. The male and female parts were cross-threaded and the connection failed when internal pressure was applied.
