Investigation Summary
At approximately 3:00 p.m. on March 9, 1991, Employee #1 was looking into a steel pipe to see what areas required patching. As he turned from looking inside the pipe, he was struck by a backing forklift and crushed between the pipe and the forklift's right front tire or fender. Employee #1 was airlifted to San Bernardino (CA) County Hospital, where he died on March 31, 1991.
