Employee injured in fall when ladder slips out

At approximately 2:30 p.m. on January 7, 1991, Employee #1, a construction foreman, had finished work on the first level roof of an old house. He then wanted to go up to the second level roof and placed a three-section portable wooden ladder against the 20 ft tall wall supporting the roof. When he had climbed to the next to last rung, the ladder slipped back and Employee #1 fell, initially striking the first level roof and then flipping onto the ground. He was hospitalized for 4 days for treatment of chin lacerations and a broken right leg. Employee #1, who had 6 years of experience with the employer, had not tied or blocked the ladder to keep it from slipping. The employer was cited for a serious violation of T8CCR 1675(j).
