Investigation Summary
At approximately 9:55 p.m. on October 16, 1990, Employee #1 was standing on the 9 to 10 in. wide shroud of a CFL-80C2 aircraft engine fan module, loosening a star bolt in order to remove the fairing. Her wrench slipped from the head of the bolt, causing her to lose her balance. Employee #1 fell 3 to 4 ft onto the cement floor and suffered a compound fracture of her left wrist and a dislocated left elbow.
