Employee's foot injured when struck by dislodged object

At approximately 12:30 p.m. on February 13 1991, Employee #1 was repairing a ring-type header device used in the manufacture of large diameter steel pipe. The header was on the ground near the path of a wheel-mounted mobile crane. The crane operator misjudged the clearance between the header and the crane, and struck the header. The dislodged header struck Employee #1 on his left foot. He sustained injuries that required hospitalization.
