Investigation Summary
At 11:35 a.m. on January 24, 1991, Employee #1 was dadoing on a Delta Rockwell table saw, a machine that met the definition of a class B manual feed rip saw. He and a helper were cutting a 3/8 in. slot in a 4 in. by 4 in. by 3 ft long piece of pine. As the helper removed the wood from the down feed side of the saw, Employee #1 lost his balance and used his right hand to try to steady himself. His right little finger contacted the dado blade and was amputated. Employee #1 had been using a piece of the wood as a push stick.
