Investigation Summary
At approximately 1:15 p.m. on November 26, 1990, Employee #1 was riding a short distance, 100 to 200 yards, on the back of a flatbed pickup truck driven by a coworker who had offered him a ride. The pickup had a proper bench seat on the back adjacent to the cab; the passenger seat was empty. As the truck made a slight turn at approximately 5 to 7 mph, Employee #1 fell off, a distance of about 3 ft. He suffered injuries that required hospitalization.
