Employee's finger amputated in paper cup-making machine

Employee #1 was a machine servicer for Sweetheart Cup Company, Inc. The #334 Gem machine was jammed, and a coworker locked out the machine to adjust it. After making the adjustment, the coworker jogged the machine to check it. Employee #1 noticed a piece of paper remaining in the machine and tried to grab it with her right hand. One of her fingers was amputated by the machine's paper cutter.
