Investigation Summary
On February 11, 2003, an employee of Grimmway Farms was working on bean cutter number 5. The employee attempted to clean some carrots out of the bean cutter with the machine still running. He did not de energize the machine and lock it out. This resulted in his right hand thumb tip being amputated. He was taken to Southern CA. Orthopedic Institute for emergency treatment where he was treated for amputation but was not hospitalized.
