Investigation Summary
At about 3:10 p.m. on January 31, 1992, at NSI Technology Services Corp., Employee #1 was using a table saw to shape a small piece of wood that was about 4 in. long by 3 in. wide. He was holding the wood in his right hand without using a push-stick, and the saw blade was not guarded. Two of Employee #1's fingers were amputated by the running saw blade and a third finger was seriously cut. He was hospitalized, treated, and released on the same day.
