Investigation Summary

Investigation Nr: 170623649
Event: 08/07/1996
Employee injured when struck in face by disk grinder

On August 7, 1996, Employee #1 was preparing to weld a partially broken skid to the bottom of a 144 cubic ft bin used to store aluminum cans. He was using a 3.5 HP Skil professional disk grinder, model 9645, to grind a weld on the storage bin. The grinder was missing a side handle, so Employee #1 supported it by holding the rear handle with one hand and the bottom of it with the other hand. He lost control of the grinder when it became hung up and suddenly kicked back, striking him in the face. Employee #1 suffered a lacerated chin and the loss of four teeth. This accident resulted from the grinder missing a side handle, which would have enabled the operator to control it.
