Employee's arm amputated by saw blade

Employee #1 was cutting 1 in. by 4 in. boards into 36 in. lengths with a radial saw. He had allegedly been doing this particular job for three days and had been operating this saw for 1 1/2 to 2 weeks since he was hired. Employee #1 claimed to have at least 1 1/2 years of experience with an identical or similar saw. For some reason, he reached behind the running saw blade with his left hand while his right hand pulled the saw forward to make room for the left hand. The saw blade bit into the 1 by 4s that he had already cut, causing the saw to jump and strike his chest. Employee #1 lost control of the saw, which moved back to its original position, amputating his left arm.
