Employee injured by furnace walking beam

At approximately 3:15 p.m. on March 30, 1992, Employee #1 was cleaning in a water enclosure of the #2 furnace owned by Oregon Steel Mills. Employee #1 worked for Valley Systems, Inc., a subcontractor for the furnace cleaning. An employee of the contractor activated the walking beam component of the furnace. Employee #1 sustained injuries for which he was hospitalized. The furnace was not properly locked or tagged to prevent movement.
