Employee killed by overturning forklift

At about 4:26 p.m. on August 18, 1991, Employee #1, who had not been assigned to work, arrived at his place of employment. He mounted a parked forklift, and drove it around the building complex. When Employee #1 entered the parking lot's driveway with the forks fully elevated, the forklift tipped over, crushing and killing Employee #1. There were no witnesses to the accident.
