Investigation Summary
Two bridge cranes ran in parallel bays of a warehouse. At one end of the crane rails, a catwalk was fastened between the rails for a maintenance work platform. Employee #1 and coworkers were on the catwalk to check on noises being made by one crane, when she was struck by one of the bridge ends of the second crane and pinned against a support column. He sustained a fractured elbow and severe lacerations to his buttocks that required a temporary colostomy. Before getting on the catwalk, the employees did not lockout the second crane or notify its operator of their presence.
