EMPLOYEE INJURED WHEN HER SHOE HEEL BECOMES CAUGHT

AT APPROXIMATELY 3:30 P.M. ON MAY 4, 1993, EMPLOYEE #1 WAS "DROPPING" STOCK (I.E., MOVING STOCK TO RACKS FROM A FORKLIFT) ON THE BOTTOM TWO LEVELS OF THE RACKS. EMPLOYEE #1 BACKED UP TOO CLOSE TO THE STURDILY-BUILT RACK BEHIND HER. EMPLOYEE #1 WAS NOT AWARE THAT HER RIGHT HEEL WAS EXTENDED PAST THE REAR OF THE RAYMOND FORKLIFT. WHEN EMPLOYEE #1 TURNED, HER HEEL WAS CAUGHT BETWEEN A VERTICAL MEMBER OF THE RACK AND THE FORKLIFT.
