Employee's fingertips amputated in punch press

At approximately 12:00 noon on January 8, 1992, Employee #1, a punch operator at A & H Steel, Inc., Union City, CA, was operating punch press machine #719. He was making 4 in. cap bases out of #26-gauge aluminum sheet metal. Employee #1 states that he pushed the STOP button with his elbow. Thinking the machine was off, he stuck his hand into the die closure area to remove some scrap metal. The die shear closed, amputating the tips of his right index, middle, and ring fingers.
