Employee's fingers amputated in meat blender

At approximately 7:00 p.m. on August 30, 1991, Employee #1, of Farmland, Inc., was cleaning a 5,000 pound meat blender. He had observed the process twice, but this was his first solo cleaning of the blender. The cleaning process includes hosing down the blender to wash the residue out of the doors. There is a guard over the doors, but the process includes cleaning behind the guard. When Employee #1 opened the door to extract some meat, the blades of the blender caught his fingers and the index and middle fingers of the right hand were amputated.
