Employee killed when caught in conveyor nip point

At about 10 a.m. on April 23, 1992, Employee #1, of Leo Shavings, Inc., was discovered by an employee of Harbor Pallet Co. in the conveyor pit with the upper part of his body and head caught between the tail pulley and conveyor belt. The paramedics were called and pronounced Employee #1 dead upon their arrival. Employee #1 had been operating the wood grinder alone and no one knew why he was in the conveyor pit, which is about 6 feet deep. The Anaheim Fire Department, which responded to the scene of the accident, discovered a job-built wood ladder accessing the pit. It is possible that Employee #1 went into the pit to remove fallen scrap wood. At the time of the accident the nip point of the conveyor belt and tail pulley was not guarded and the machine was running. There is a strong possibility that the cause of the accident was the unguarded nip point. The employer is being cited for a serious violation. The company president installed a lockout program and he said that he had instructed the employee to shut off the machine and padlock the power source before going in the conveyor pit.
