Investigation Summary

At approximately 1:30 p.m. on October 10, 1991, a tractor driver was driving a spotting tractor on the north side (shipping) of the Lucky Stores grocery warehouse, pulling his tractor out in front of the parked customers' trailers. At the same time, Employee #1, driving a multi-purpose cart, was traveling eastbound. They collided, throwing Employee #1 15 feet from his cart. The paramedics were called and took Employee #1 to Mission Hospital, where he was treated for lacerations to the back of his head, facial cuts and bruises, and possible fractured ribs. He was not yet back to work during this investigation. He is reportedly suffering from memory lapses. Photographs were taken by the Lucky Stores safety engineer, who was at the site during the accident. He said that the point of impact was 15 feet away from the parked customers' trailers, which indicated that there was no blind spot. The front bumper of the spotting tractor hit the right front and side of the cart. The tractor driver said that he did not see the cart. He said that he was following the yard's speed limit of 10 mph. There was no violation noted in connection with this accident.
