Employee's finger amputated in machine

Employee #1 was operating a machine that forms, fills, and seals containers of intravenous solutions. He placed the saddle parts of the containers into the machine at his work station. They were not aligning properly as they moved along and Employee #1, in correcting this, placed his hand inside the machine. He suffered a partial amputation of his left index finger.
